Open for all players, all ages. Everyone must sign the waiver form for liability at the venue. IHSA athletes must check IHSA rules.

FORMAT: ABCD Drop Flight or Round Robin
Minimum of 4 entries per event and maximum of 32 entries. BWF 3 x 21 rally point scoring for all matches 
(for details, visit http://www.usabadminton.org and click on "Rules"). Other formats such as round robin may take place if the event has less than 8 entries.

Friday, May 22nd 
5:00 PM Registration and Warm-Up
6:00 PM Men’s & Women’s Singles

Saturday, May 23rd 
7:30 AM Registration
8:00 AM Men’s & Women’s Doubles
11:30 AM Presentation of Susan Jurkonis Award
Lunch Break till 12:30 PM
12:30 PM Mixed Doubles

PRIZES: T-shirts for all entries postmarked prior to May 8th. Trophies and/or prizes for all flight winners and runners up.

COST: $20 for first event and $10 each additional

You must MAIL the payment after this online entry to complete registration.
You are NOT registered until we receive payment.
Mail must be postmarked on or before 05/16/09. 
Late applicants may not be accepted if event is full and we will randomly assign your first round opponent. 
Each player must submit their own entry form; entries for doubles are considered 'received' upon receipt of BOTH partners' entries.
Payment MUST accompany entry form (checks payable to John Brander).
